What they do
Bioventus sells a portfolio grouped into three businesses: Pain Treatments & PRP (knee osteoarthritis HA injections, the distributed XCELL PRP system and peripheral nerve stimulation), Surgical Solutions (Ultrasonics bone resection and Bone Graft Substitutes) and Restorative Therapies (Fracture Care, including low-intensity pulse ultrasound). It reports through two geographic segments, U.S. and International, which were 88% and 12% of fiscal 2025 net sales respectively. Products are used across knee, spine, trauma, neurosurgery, general surgery and other clinical areas in physician offices, clinics and hospital settings.
Revenue drivers
- Pain Treatments & PRP — Includes Durolane and other intra-articular HA injections plus the distributed XCELL PRP system; the largest business, at $81.7 million of Q2 2026 net sales, up 11.5%.
- Surgical Solutions — Ultrasonics (BoneScalpel and neXus platforms) and Bone Graft Substitutes; $50.4 million of Q2 2026 net sales, down 4.5%.
- Restorative Therapies — Fracture Care, principally the EXOGEN Bone Stimulation System; $21.1 million of Q2 2026 net sales, down 2.4%.
- U.S. versus International — The U.S. segment was 88% of fiscal 2025 net sales and International 12%, with international growth pursued through direct and distributor channels.
Recent performance
Second quarter 2026 worldwide revenue was $153.2 million, up 3.8% as reported, with double-digit growth in Pain Treatments. Net income attributable to Bioventus was $33.4 million versus $7.5 million a year earlier, aided by removal of a $24.6 million deferred tax asset valuation allowance; GAAP diluted EPS was $0.47 versus $0.11, and non-GAAP EPS was $0.22 versus $0.21. Adjusted EBITDA was $35.3 million, up 4% from $33.8 million. Cash from operations in the quarter was $19.9 million. For full year 2025, net sales were $568.1 million, net income $27.3 million and operating cash flow $74.7 million.
Strategy
Management describes a four-pronged strategy: strengthen leading positions in HA, BGS and Fracture Care; invest in emerging technologies including PNS and PRP; expand the Ultrasonics platform; and grow international markets. The XCELL PRP System was fully launched in the U.S. in August 2025 and is sold through the existing HA sales force, and TalisMann and StimTrial received FDA 510(k) clearances in July 2025. On August 5, 2026, the Board formed a committee of independent directors and initiated a review of strategic alternatives, citing external interest. The company reaffirmed full-year 2026 guidance for revenue, Adjusted Diluted EPS and cash from operations.
Risks
- Credit agreement covenants — The 2025 Credit Agreement requires a minimum interest coverage ratio and a maximum consolidated total net leverage ratio, and failure to comply could trigger an event of default.
- Segment softness — Surgical Solutions fell 4.5% and Restorative Therapies fell 2.4% in Q2 2026, with Ultrasonics capital placements and international orders shifting to the second half.
- Customer mix pressure — Restorative Therapies Q2 2026 revenue declined on a change in customer mix, specifically Medicare patients, for the EXOGEN Bone Stimulation System.
- Legacy litigation — The Ciarciello putative class action settled for $15.3 million with final approval entered December 18, 2024, while the related Grogan and Sanderson derivative suits remain stayed or pending.
Outlook
Bioventus reaffirmed its full-year 2026 guidance for revenue, Adjusted Diluted EPS and cash from operations in its August 5, 2026 release. Management said it remains focused on disciplined execution and continued investment in its four growth drivers, expecting that combination to accelerate revenue growth, strengthen profitability and drive significant free cash flow. The Board has also begun a review of strategic alternatives while maintaining confidence in the business and management team.
